Support Services for Seniors and Vulnerable Persons
The CSCHN is mobile, regularly visiting retirement homes and seniors’ clubs in the Hamilton and Niagara areas to deliver support services.
These services are defined based on the priorities established by the Ministry of Health and Long-Term Care and the Local Health Integration Network in order to improve the health of seniors in our communities. The program aims to prevent falls and reduce emergency room visits through:
- Individual home visits for priority clients
- Group activities: prevention, promoting healthy living and self-management

To make its services more accessible, the CSCHN offers home visits for clients with priority need.
Home visits take place between 7:00 am and 7:00 pm, Monday to Friday.
Eligibility: Home care services are provided in French to clients who meet the eligibility criteria. Clients may be referred by their family doctor, regardless of whether the doctor practises at the CSCHN, by CSCHN personnel or by a member of the community.
Eligibility criteria: Home care services are available to individuals who are:
- French-speakers or agree to be served in French
- Aged 55 or over, or 18 or over and considered vulnerable
- Infirm, limited or dependent
- Affected by reduced mobility
- Underprivileged or disadvantaged
- Caregivers to someone who meets any of the above criteria

Case management services:
- Assess clients’ health needs
- Help clients understand and navigate the health system
- Inform clients and provide support in making decisions about long-term care or power of attorney
Case managers may also refer clients to health or community services, as needed.
Clients can meet with a CSCHN Case Manager at our offices or in their own home.
The role of our Senior Care Social Worker is focused on individual support and group activities:
- Organizing activities in the community
- Providing support services
- Organizing workshops and activities to raise awareness of physical and mental health issues
- Organizing and coordinating volunteers for the palliative care program, which supports people in the final stage of life
- Coordinating volunteers for friendly visits, grocery shopping or other activities with seniors or vulnerable persons
- Organizing consultation meetings with the advisory committee for seniors to better respond to real issues in the community
