What we do
We provide patients of primary care providers in private practice with brief, solution-focused mental health services.
Who we are
We are a team of registered mental health clinicians and mental health outreach workers.
Where we work
We have a single point of access/central referral process. Our services are provided in multiple locations across the Niagara region. Our team works out of community health centres, but we are also available to provide services in community settings or physician offices.
The services we provide
We provide evidence based treatment and intervention in both individual and group formats at no cost.
- Psychotherapy and counselling
- Assessment and care plans, developed in collaboration with patients
- Case management
- Advocacy, system navigation and connection to community resources
Referral Criteria
Referral must be completed by a primary care provider
Referral does not require a psychiatric diagnosis
Patient must be 12 years of age or older (under 18 must go through Contact Niagara)
Mild-moderate mental health
(Provide interim service for patients awaiting secondary level mental health services)
