Who are we?
We are a team of registered mental health therapists and mental health caseworkers.
What do we do?
We provide patients of primary care providers in private practice with brief, solution-focused mental health services.
Where do we work?
We have a single point of access/central referral process. Our services are provided in multiple locations across the Niagara region: CSCHN, QUEST, Heart Niagara as well as Bridges in Fort Erie and Port Colborne.
What services do we provide?
We provide evidence based treatment and intervention in both individual and group formats at no cost.
Short-term psychotherapy and counselling, assessment and care plans developed in collaboration with patients, case management, advocacy, system navigation and connection to community resources.
Who can we help?
Referral must be completed by a primary care provider (Family Doctor/NP in private practice, CHC and FHT not eligible) Referral does not require a psychiatric diagnosis Mild-moderate mental health needs
